REGISTER AS A NEW PATIENT
Before your visit:
In preparation for your visit, you can download and fill out the patient forms before your appointment.

Along with the patient forms, kindly bring the following with you:
1. Driver's license or a government issued photo ID.
2. Your insurance card(s) and any co-payment due for the visit.
3. Referring physician's / PCP referral form if your insurance requires. Please note it is the patient responsibility to ensure proper referrals are in place should your insurance plan require it.
4. An updated medication list. We will make a copy of your list.
5. If applicable, please bring CD and report of prior CT scans, chest X-rays or any other pertinent information.

In case you are not able to make your scheduled appointment, please call our office at 480-722-7589 at least 24 hours in advance in order to avoid a $40 non-cancellation charge.
